don't use BOTH item lev AND legendary level...try leaving level alone (0) and pick a different "flag"...oh, and id suggest not messing with the teir quality until you get them to come out how you want them consistently...adding unnecessary steps and complications will just make it harder for you to isolate and correct the settings causing the issues (i had the same problems when i started) and i don't know if you've done a thorough read through of the discussions here, but there is a lot of valuable information, it can just be hard to find
